After we get to know you and understand your hopes and dreams, our first goal will be to give you the information you need to make informed decisions about your journey to parenthood.

Making the decision to seek fertility treatment can be a large emotional commitment and can seem overwhelming at times. At SIRM St. Louis, you will be under the care of one of our reproductive endocrinologists, who have extensive experience in the field of reproductive medicine and fertility treatments. Our physicians have a firm commitment in providing the highest level of personal attention to each individual patient’s infertility care and helping them achieve their dream of a family.  In addition to the compassion and expertise that our team provides, we have put the processes in place to make sure that when you are ready to make the commitment, we are here to make the experience the best it can possibly be.

SIRM St. Louis provides skilled and experienced support staff who are available to help you understand the fertility treatment processes as well as your insurance coverage and potential out of pocket expenses. We will work closely with your insurance company to best understand how your benefits can be applied to the diagnosis and treatment of infertility. In most cases, we are able to bill office visits and diagnostic test to your insurance company.

Please read through and complete the New Patient Packet (click here to download) to the best of your ability.

Please send the completed packet to SIRMMOPatientServices@sherinstitute.com as soon as possible, at least 72 hours prior to your appointment.

In addition, we request the following at the time of your appointment:

  • A copy of insurance card (front and back)
  • Any pertinent medical records
  • A copy of a valid photo ID (e.g. driver’s license, passport, etc.) for you and your partner (if applicable)

Please be advised that any costs for the consultation and services rendered are the responsibility of the patient.  If the information in the New Patient Packet is not completed and submitted in the time requested, and insurance verification is unable to be processed prior to the appointment time, patients will be responsible for the consultation fee at the time of the appointment.
